Intruiged about why the recruitment process for this role required a "full medical"...is the Chairman expected to set an example by playing in The Hundred?!

Being ever sceptical about the ECBs objectives here: it would seem useful to them to have an ex Surrey Official in this post, a handy leaver on Surrey any time they object to an ECB policy perhaps? I can't see them employing him if he is not on board with the 100 or indeed with a reduction in the Championship which should be everyone's concern. And as the ECB only seen to be concerned with "excellence" and promoting the 100, what is his view on the future of grass roots cricket?
As for him passing a fitness test, well I can only think he will be captaining the new Pensioners 100 Covid Free inauragal competion next year or they need to ensure he doesn't keel over with the pressures of the job. Rather bizarre otherwise.

... the medical will probably be for insurance purposes.

No doubt it will, but what exactly does a "full.medical" entail, and - assuming it is as rigorous as it implies - why would an insurer require it of a 55 year old going into a senior administrative role?
